The red death plague is a manifestation of the Red Death in disease form. At the tolling of the ebony clock, the red death plague afflicts all those within Prospero Hall. Guests always die whereas Prince Prospero and his servants "merely" suffer its effects for a short while.[1] The ebony clock can release a weaker form of the disease as a defense mechanism against attackers.[2]
