A mealy-eyes[1] is the undead spirit of a person who awoke to find himself buried alive and died while conscious. They are also known as worm-kings because of their crowns made of writhing earthworms and eye sockets filled with mealy-worms. Like most forms of undead, the mealy-eyes has a hate for the living, but it prefers to play this out via trickery and its illusion abilities over brute force.

Mealy-eyes often have a good affinity with the Sons of Kyuss. Note that the mealy-eyes was never updated to 3.x Edition, so the creature types assigned below are speculative.
