A cult leader may or may not be a religious leader. One that is a religious leaders differs from a spiritual leader in that he does not exercise significant influence over the people of one or more lands. The high priest of the Cult of the Morninglord is a cult leader as he has no official standing in Barovia and dare not overtly shape events in that land.
